March 11 - 15, 2013 ~~ 13th Week


This week we just focused on the distribution of memo for the change location of PIT Workshop because on the first venue the PGNT's were going to have their examination that's why we need to look for another vnue. The event was fully organize and it ends successfully, it's all about the lean six sigma for the company, for the further improvement. Our department held this event, so that the staff nurses are ready for their more challenging role in the hospital.

March 5 - March 8 ~~ 12th Week)

We focused on the distribution of policies, quality procedure and monitoring of the nurses if they already answered the online survey. we distribute the policy entitled Safety evacuation of patients during emergencies and hand hygiene and a quality procedure entitled processing of requests for liver disease and transplant center. 
I also edit the presentation for empowerment for their video making contest.


February 25 - March 1 ~~ 11th Week

This week we still working with the emails of those identified associates of each nursing units, and while we are follow up their emails we also distribute the policy entitled Training equipment reservation and infection control consultation.After we also need to segregate the superseded policy and forward it at Audit management and services department.

February 18-22, 2013 ~~ 10th Week



Distributing the four policies are the main task of mine this
week. The title of the four policies are Corporate
Giveways, Blood transfusion Committee, Transporting of
Carts, Wheelchairs, Bed, Food Conveyors and other
portables equipment and the Consultations at the
Employees Health Clinic.
I also distribute the Forms Summit memo to the three
clusters the OR, MCN, and the Intensive. The memo was
need to distribute as soon as possible because those
clusters are need to inform for the Forms Summit on Global
City.
I also encode the DON Policy, but it almost has a three
revisions, actually I really didn't focused to what my
supervisor instructed because at that moment I have a
Disminoria, but then despite of that I finished the task
that she gave until it becomes perfect.

February 11-15,2013 ~~ 9th Week


This week I experienced to audit some equipment from MS2
cluster, with those patients room. While we we're auditing
those tools I observed that all the nurses from different
units really do their respective jobs, how they interact

with the patients its really nice to see the good service
that they gave. The equipment that we audit was the space
pump, kangaroo, IVAC, Space pump adaptor/pole clamp and
the space station we do the audit to know the originating
location of those tools because some of it are transfer to
the other units. After auditing those equipment we need to
follow up the Iip action plans and list of identified
associates for the Iip diagnostics assessment
organizational climate survey of each nursing units. Then
when we collect their e-mail address we need to encode all
of it.
Today we also distribute the policy entitled "Approving
Authority for Executives" to the 32 units of nursing.

February 4-8,2013 ~~ Eight Week



We continue to update the policies and we distribute thenewly arrive policy and while distributing the policies I also

need to follow up the article for Gazette and only two
nurses submit their article.
I also sort and distribute the work instructions to the 32
units of nurses. We also assist our boss about his project/
activity which is the PIT Workshop, only few nurses were
invited . It has a 45 participants. The talks was all about
the quality, that we need to look for the quality of the
work not just doing it but by putting a great/quality on it.
The workshop done successfully.

January 28-31,2013 ~~Seventh Week


Tis week we focused to update the policy. We need to put
the new label on the folders and to update the title of
policy on the orientation sheet. We finished to update all
the policy folders in different folder we need to pull out
the supersede and canceled policy.
We also distribute the memos for PIT Workshop to the
different units, and while we we're distributing the memos,
we also follow up the article for Gazette, but only one
nurse finished it on a deadline, because all of the nurses
was busy with their duty. Next week I'm going to followup
those article.

January 21-25,2013 ~~ Sixth Week



We update all the policies, we remove from the folder all
the canceled and superseded, and we also put the title
document on the latest one. We only finished updating five
folders, that's why we need to continue it next week. We
also replaced the old label of those folder.
My boss also asked me to went through the 32 units to
gave the article for the essay writing, because the Vice
President asked them to make an article for the Gazette,
then she will going to choose the best one.
We contacted the 32units of nursing to update them that
the PIT projects was resched, from Feb 7 to feb 6. This is
also a contest for the nurses.
This week I didn't experienced to Audit, because my boss
needs to finished all his reports and those reports are
confidential, like the fall incident that's why he can't asked
me to help him.

Fifth Week ~~ January 14-18, 2013 


   Auditing the planned and unplanned discharge was my task this week. Auditing the time was the most important then everything follows, like the turn-in, the discharge summary, discharge orientation, the inpatient evaluation form (PEF) and the gate pass, sometimes there's are more than five patients discharge. There are different kinds of payments: the individual,express VIP, and the HMO.

Sorting the Policy and accepting telephone calls.

       My boss was invited to lecture the nurses about the Joint Commission International Accreditation (JCIA). It says that every day is a JCIA day, because it’s not the popularity but the service that they gave to their patient and the trust that the patient gave to them.     

       We also segregate different policies, after we need to distribute it to the 32 units of nurses. Policies are very important because it is the baseline of every act in the hospital.

Fourth Week ~~ January 7-11, 2013  

            
              After a long vacation, this was our first week to be in the office. The things that I've done this week were like the things that I made in my previous weeks. We distributed the policies to the 32 units of nurses. I also computed the result of the multidisciplinary initial assessment database (MIAD) most of the nurses got a perfect rating, but still there is a unit of nurses that acquire less than a hundred percent. Then after computing, I made an analysis and recommendation of the multidisciplinary initial assessment database (MIAD), functional assessment record (FAR) and the nutrition risk screening (NRS).

Third Week ~~ December 17-21, 2012



      This week was very interesting, because I experienced to audit some units of nurses with my boss. The first thing that we did was to gather all the tools that we are going to used. That is why we have the nutrition risk screening, multidisciplinary initial assessment database and functional assessment record, this are the tools that we're going to use for auditing. Before we went to the different units of nurses, I asked my boss if he would let me take some pictures while auditing, but sad to say he didn't let me, that's why I just take a pictures of the units after auditing. The first unit that we audited was the orthopedic care unit, when we went to the unit it seems that most of the nurses were surprised of our arrival. Upon our arrival, the nurses gave us their census; they have 26 patients and out of the 26 patient we need to audit five patients charts randomly. The orthopedic care unit did not have a perfect rating, because they did not comply with the needed requirements.
       The next unit was the pulmonary care unit, this unit was rated with a perfect rating because the nurse manager of this unit have a tool by herself in checking the patients charts, so if ever there is an audit at their unit they are ready and confident that they are going to have the perfect rating.
I just observed that in auditing even if it’s exciting to do, still it is not as easy as what I have expected. By doing this, we can see the nurses’ output are performed properly or not. 


Second Week ~~ December 10-14, 2012 

Research Systems Management formerly Quality Assurance was the division that I was assigned. My boss is the one that is in charge in auditing the 32 units of nurses. After auditing the nurses’ units, he needs to make an analysis and recommendation report out of it. He already finished auditing some nurses units. He allowed me to do the analysis and recommendation of the result of the auditing he made and gladly, I made it properly.
My second week as a trainee at St. Lukes Medical Center.


The 32 units have four clusters: the maternity and child nursing unit (MCNU), the Medical Surgical (MS) and the Intensive Unit (IU).
           To pass through the audit those units must have the Nursing Care Plan (NCP), some units got a perfect rating yet some did not. After the analysis and recommendation we need to construct a cover letter for the audit result.
       He also handles fall incidents. The three main common fall incident are: the clinical process/procedure, communications, and behavior. Every single detail of fall incident should be reported. Safety Learning Event Analysis (SLEA) is the document that contains those report 

My first week ~~  December 03 - 06, 2012

     During my first week as a trainee at St.Luke’s Medical Center, I really felt nervous. It is the fact that we are not just simply going to encounter regular staff or employee in our division, but the manager and also the vice president of the Nursing department as well.
     The first thing that they asked me to do was to count the number of votes in the internal feedback form for the employee and for the company. Before I forget the first thing that they told us was the proper telephone manner in the office, and now we are confident to answer those calls. They also discussed with us the policies, work instructions and quality procedures. Those documents are needed to be distributed to all 32 units of Nursing department. This is necessary for them to know the changes in the policies. Before circulating the documents to the different units, we must give it first to the authorized persons for signing. I have found out that policies must be revised every three years.
     In general, I did enjoy my first week.
